Roze Room is a leading provider of Hospice and Palliative Care, celebrating 25 years of service to Southern California communities.:: Roze Room Hospice is seeking a Full Time Salaried: Quality Assurance (QA) Director/Manager: Salary: $80,000.00 - $115,000.00 per year: Hospice Experience in QA preferred.: RN or LVN preferred.: Culver City, CA: Job Title: DIRECTOR OF QA (QUALITY INITIATIVES PROGRAM DEVELOPMENT) Position Summary: The Director of Quality Initiatives Program Development fills an executive full time position with overall responsibility for the QAPI program and development and implementation of new programs at all Roze Room Hospice agencies. He/she oversees review of hospice medical records and other documentation for assessment of the quality of clinical services and adherence to all policies and procedures and applicable standards, and makes recommendations for changes in policies, procedures, systems and staff education. He/she assists the CEO in evaluating and implementing new programs consistent with the organization’s mission, goals, objectives, and policies and procedures established by the Governing Board/Executives. REPORTS TO: Administrator (QA)/Chief Executive Officer (Program Development) SUPERVISES: Quality Assessment Assistant QUALIFICATIONS: Education: Graduate of an accredited school of nursing with baccalaureate or higher degree in Nursing or another health-related field Credentials: Current license in California as an RN or LVN Experience: At least three years of varied healthcare work experience as a licensed nurse, with at least one year in a Quality Assessment/Performance Improvement role. Minimum of one-year hospice experience. Core Competencies: Ability to make meaningful observations and write comprehensive accurate reports. Knowledge of QAPI functions, Medicare Conditions of Participation, applicable laws, state regulations, and standards. Demonstrated computer proficiency and knowledge of basic Microsoft applications. Ability to be attentive to detail and demonstrate analytical thought process. Ability to organize, delegate, supervise and evaluate staff. Possesses excellent verbal and written skills. Possesses a sympathetic attitude toward care of the hospice patient and demonstrates positive communication skills in interacting with all members of the hospice team. Ability to evaluate each individual, unique program to determine system changes necessary for improvement. Other: Valid driver's license, and auto insurance, reliable/well maintained transportation. GENERAL ESSENTIAL FUNCTIONS RESPONSIBILITIES: 1. Works with individual Agency Administrators and Directors of Patient Care Services to provide an ongoing, comprehensive assessment of the quality of care and services provided 2. Coordinates the implementation of the program’s Quality Assessment and Performance Improvement Plan to assess the quality and appropriateness of patient care 3. Works to assure that the Quality Assessment and Performance Improvement Program (QAPI) is in compliance with the Medicare Conditions of Participation and other regulatory agency requirements, including Joint Commission. 4. Coordinates all QAPI audits and studies as indicated in the plan 5. Develops and implements problem focused audits. 6. Participates in the hiring, orientation, on-going training, retention and supervision of appropriate and qualified QA staff 7. Identifies patterns and trends and reports them to the QAPI committee 8. Conducts quarterly QAPI meetings, reports findings and coordinates development of action plans 9. Utilizes problem-solving techniques to address areas of non-compliance 10. Maintains current information and requirements related to applicable laws, regulations and standards 11. Assures the program is reviewed at least annually. Included is an evaluation of all services that were provided and patients/caregivers response to those services. Attention is given to: symptom management, pain control, stress management, continuity of care and inpatient care 12. Assists in formulating, developing and writing hospice policies and procedures 13. Assists in orientation of new employees at all agencies 14. Assesses organizational needs and provides appropriate justification for, and benefits of, potential new programs 15. Assists the CEO in program planning and development . Reports to management on QAPI, concerns and needs. 16. Assists in evaluation of alternatives for new programs/processes to meet changing hospice needs 17. Coordinates implementation of new programs at all Roze Room Hospice agencies, as deemed appropriate by the CEO/Governing Board 18. Maintains an open line of communication with Administrators and DPCS’s at all agencies, as well as CEO/Governing Board 19. Serves as a role model and sets an example for other members of the hospice team 20. Maintains continuing education through academic studies, seminars, and workshops 21. Attends, participates, and provides in-services, staff meetings and case conferences 22. Completes all documentation in an accurate and legible manner 23. Complies with hospice policies and procedures, laws and regulations and standards of practice 24. Recommends and participates in development of at least 3 projects each year to improve agencies. 25.
