Job Details

We're seeking a Licensed Practical Nurse (LPN/LVN) who will be accountable for providing and documenting proficient nursing care, supervised by a Registered Nurse. The care should align with the patient's individualized care plan and physician's orders, while maintaining adherence to confidentiality standards and professional boundaries at all times.

Key Job Functions

- Deliver and coordinate quality patient care based on physician orders.
- Continually observe, assess, and report changes in the patient's condition and care needs to the supervisor and/or physician.
- Document actions, assessments, responses, and care coordination accurately and promptly whenever care is provided.
- Participate in, execute, and review the nursing care plan.
- Take necessary nursing action based on assessments to achieve expected outcomes.
- Identify changes in patient requirements and responses demanding intervention and initiate care to prevent or mitigate risk.
- Uphold responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Implement care using infection control measures that protect both staff and the patient according to OSHA standards.
- Educate the patient and family about disease progression, self-care techniques, prevention strategies, and meeting the patient's nursing needs.
- Keep abreast of competencies related to the nursing profession by engaging in education programs, continued education units, internal learning management skills, and skill evaluations.

Requirements

- Graduation from an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse.
- Current certification in CPR.
- Demonstrated aptitude in clinical assessments, documentation, and adherence to nursing care and policies and procedures.

Preferences
 
- A minimum of six months of recent experience as a Licensed Nurse in a clinical care setting.
- Experience in home health.

Physical Requisites

- Capability to speak, write, read and understand English.
- Ability to travel.
- Ability to lift 50 pounds.
- Ability to reposition patients and move equipment without assistance.
- Ability to withstand prolonged walking, standing, bending, kneeling, reaching, twisting.
- Can climb stairs and sit.
- Possess visual and hearing acuity.
- Strong sense of smell and touch.
- Possess mental and physical resilience to respond to emergency situations at home or during transport.

Environment

- Capability to function in diverse environments that may involve exposure to allergens and other varying conditions.
- May be exposed to blood, bodily fluids, and infectious diseases.
