Experience Required: Experience in dialysis is preferred but not necessarily required.
Role Focus: The primary responsibilities will be centered around patient education, ensuring patients understand their dialysis treatment, procedures, and self-care.
Schedule: Full-time, Monday through Friday, 8 hours per day.
Additional Requirements: There will be an on-call rotation, which includes incentive pay.
Key Responsibilities Might Include:
Educating patients about peritoneal dialysis, including techniques, equipment, and troubleshooting.
Providing ongoing support and answering patient questions.
Desired Skills:
Strong communication skills for effective patient education.
Experience or training in dialysis is beneficial.
Ability to work independently and manage time effectively
Collaborating with healthcare teams to optimize patient care.
Assisting with the setup and maintenance of dialysis equipment.
Monitoring patient progress and adjusting care plans as needed.
PURPOSE AND SCOPE:
Functions as part of the Home Therapies dialysis health care team. Ensures provision of quality home patient care in accordance with company policies, procedures and training. Supports the company’s commitment to the Quality Enhancement Program (QEP) and CQI activities, including those related to patient satisfaction. Actively participates in process improvement activities that enhance the likelihood that patients will achieve the company’s Quality Enhancement Goals (QEP).
