At Sunrise, we have an opening for a Wellness Nurse, a role that is pivotal for ensuring the overall health and wellness of our residents.

Role Overview:

The Wellness Nurse has a broad and diverse set of responsibilities including:

- Identifying the continuous needs and services for residents, working to deliver the highest standards of resident services.
- Ensuring appropriate follow-up, assessment, and documentation for any changes in the healthcare services and status of residents.
- Assisting the Resident Care Director (RCD) in conducting assessments of residents at the time of move-in, 30 days post-move in, semi-annually, or whenever there is a change in status.
- Conducting monthly wellness visits for all residents.
- Monitoring and following up on incidents where residents may have experienced a fall or other incident and documenting and reporting as necessary.

The Wellness Nurse also takes the lead in maintaining regular communication with the resident's family or responsible party, contacting the resident's attending physician when necessary. They are responsible for overseeing the general medical health of the residents, ensuring monthly checks for weights, vital signs, etc.

In addition, they assist in maintaining wellness files in accordance with Sunrise policies and state and federal regulations, and are responsible for the maintenance of medical supplies and emergency kits.

Job Qualifications:

The ideal candidate should have:

- Graduated from an approved nursing school/college.
- A current state license as a professional Registered Nurse (RN).
- At least one (1) year experience in home health, assisted living, hospital, or long-term care environment.
- Comprehensive knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.

Furthermore, they should be well versed in federal, state, and local regulations. Excellent assessment skills, the ability to handle multiple priorities, written and verbal skills for effective communication, strong organizational and time management skills are also necessary for this role. Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) and Sunrise applications is also required along with the ability to learn new applications. The candidate should have the flexibility to work weekends, evenings, and flexible hours to be available to our customers on peak service delivery days and times.
