Responsibilities

As a Care Manager, your duties include:

- Creating significant connections with a specific group of seniors and their families as you offer assistance with daily activities, cater to their individual care needs, and get to know their unique personalities and preferences.
- Informing management of changes in condition and recommending adjustment in the level of care and services.
- Assisting residents in life skills as well as other enriching activities based on their individual profiles.
- Incorporating a mix of multi-sensory experiences into the resident's day.
- Participating in the development and monthly updates of Individualized Service Plans (ISP).
- Keeping track and checking in on your designated group of residents throughout your shift.
- Collaborating with the community team to ensure compliance with Occupational Safety and Health Administration (OSHA) requirements and Risk Management programs and policies.
- Responding to dining requirements of residents and guests while adhering to Sunrise’s hospitality and service standards.
- Encouraging residents to partake in life skills and enriching activities as outlined in the ISP.
- Upholding our commitment to serve our residents and guests through our Principles of Service.

Qualifications

Ideal applicants will have:

- Deep dedication and passion for serving seniors with exceptional customer service skills.
- Positive attitude, flexibility for varying duties related to resident service, and ability to work within a team.
- High School diploma or GED. 
- Where required by state regulations, applicants must have maintained certifications.
- Applicants must be at least 18 years old.
- Previous experience working with seniors is preferred.
- Ability to promote the best interests of residents in decision making.
- Effective verbal and written communication skills.
- Organization and time management skills.
- Demonstrable good judgment in problem solving and decision making.
