As a Hospice RN/ RN Case Manager, your roles entail:

- Directly reporting to the Administrator, Executive Director, or Patient Care Manager Senior and managing each patient.
- Serving as the primary liaison between the physician, the patient/family, and the hospice team.
- Provision of direct care services to hospice patients, supportive care to their family, overseeing a manageable caseload while ensuring close relationships with patients and families are formed.
- Armoring patient caregivers with the requisite knowledge for improved caregiving.
- Coordinating with a multidisciplinary team including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to provide comfort, peace, and dignity.
- Assessing patient and family needs, aiding in the creation and execution of each patient's care plan.
- Managing all assigned patient clinical records.
- Participating in the company’s quality assessment and performance improvement programs.

Eligibility Requirements for Hospice Registered Nurse / RN Case Manager:

- Empathetic, compassionate, and kind in dealings with patients and their families.
- Capable of rendering care in the patient's home setting.
- Able to maintain composure and adaptability during crisis moments.
- Possession of a valid RN license (or eligibility to obtain one).
- A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology is highly desired.
- Preferred hands-on experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.
- Current driver’s license and auto insurance.
- Up-to-date CPR certification.
