Job Summary and Qualifications

We are seeking a Discharge Planning Registered Nurse to join our Amazing Hospital Team, who will assist in managing the discharge process to make certain that patients are discharged in a timely manner & to facilitate the patient throughout and to be in compliance with guidelines and appropriations.

What you will do in this role:

    • Facilitates patient throughput by identifying pending and confirmed discharges. Uses tele tracking to enter the identified discharges when the units have not.
    • Communicates with the Patient Placement Center for direction for areas with priority for discharge. Assists those areas to expedite discharges.
    • Identifies barriers related to timely discharge and communicates appropriately. Assists with discharge paperwork as needed to facilitate timely discharge of the patient
    • Reviews with Charge Nurses anticipated discharges for the following day to assist with downgrading the patient to a lower level of care if appropriate.
    • Communicates with physicians and Case Management for anticipated discharge needs prior to the day of discharge.
    • Assists in identifying core measure patients and assures criteria have been met and completed prior to discharge.

    · Setting high standards of performance for self and others; assuming responsibility and accountability for successfully completing assignments or tasks; self-imposing standards of excellence rather than having standards imposed.

    What qualifications you will need:

    · Graduate of an accredited diploma, associates, or baccalaureate degree registered nursing program.

    · Prefer Minimum 5 years of Clinical Nurse Experience

    · Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date)

    · Current American Heart Association BLS Certification

    · Meditech knowledge helpful.

    · Excellent communication skills Needed
