Great opportunity to join an experienced Case Management team. Role focuses on transition planning and Utilization Review. RN Case Managers are assigned geographically to inpatient units and paired with a Care Team to facilitate through put.. This position is focused on Emergency Department coverage on weekend Saturday/ Sunday 8 AM - 6 PM with a goal of assessment and developement of initial transiion plan . Utilization Review is embedded in this role - team uses InterQual and provide training in use of this product. Looking for good critical thinking skills and willingness to learn in a fast paced environment. Job Specific Responsibilities: - Initial assessment and development of transition plan - Participation in multidisciplinary rounds - Utilization review - Patient advocacy - Ability to delegate tasks to Case Management assistants Salem Hospital, an affiliate of Mass General Brigham, is committed to supporting patient care, research, teaching, and service to the community. Preferred: - Case Management, Home Care Discharge Planning experience
