The Oncology Nurse Navigator (ONN) uses advanced nursing processes in decision-making, patient assessment, data analysis, development and implementation of methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. The ONN offers individualized support across the continuum of care to patients, families, and caregivers to expedite care delivery and alleviate barriers. The ONN is responsible for the coordination of care through interdisciplinary collaboration with a a goal of delivering efficient care with optimal outcomes. The ONN will ensure ease of transition through various health settings during the patient's cancer journey.
Facilitates daily coordination of patient care. Interacts with staff, patients, families, visitors, members of the health care team and external partners to develop, coordinate, and manage the plan of patient care. Must work autonomously and in a team environment. Utilize delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care. Demonstrate the role of patient advocate, protector of patient's rights, provider of psychological/social/cultural support, and tolerate stressful situations. Excellent interpersonal communication, customer service and teamwork skills required. Above average organizational skills, ability to multi-task, and attention to detail in work. Strong skill in written and verbal communication.
Education: Graduate from an accredited school of nursing BSN/MSN preferred.
Experience: Three years of experience of specialty specific nursing in a hospital, clinic, or community setting. National Nurse Navigation training preferred.
Licensure/Certifications/Registration: Current licensure as a Registered Nurse in the State of Virginia required. AHA BLS-HCP required.
Other Minimum Qualifications: Excellent written and oral communication skills. Excellent interpersonal skills, organizational ability, and initiative. Proven ability to understand medical terminology, problem solving and handle stressful situation with a professional manner. Good team dynamic skills in order to facilitate change and achievement of defined outcomes. Must be able to work with minimal amount of supervision.
