Overview
  • Provides on-site (face-to-face) and telephonic outreach, disease management and/or care management, education and other clinically-based activities for OHSU Health Services Members. Provides supportive counseling to reinforce Members’ strengths and assists them in dealing with the stress and feelings of grief, fear, and anger associated with serious illness, injury, hospitalization or disability.
  • Conducts interviews and assessments with Members and/or Member families and other community resources to gather information necessary in identifying and evaluating psychosocial, cultural and financial conditions, develops plans of care and interventions, and sets goals for plans of care and behavioral modification within his/her scope of practice.
  • Collaborates with OHSU Health Member’s medical and health community-based providers (that may include the primary care provider (PCP), specialist provider, behavioral specialists, or others as may be indicated) regarding the member’s treatment and resource needs and plan of care. The frequency of contact is determined based on the member’s risk acuity ranking and clinical need.
  • Collects clinical and engagement data and enters information into the electronic medical record (EMR) for tracking and metric purposes. Routinely documents all Member interactions and member engagements in the EMR to ensure a longitudinal record is maintained of all member interactions. Maintains medical record confidentiality at all times through the proper use of computer passwords, maintenance of secure files; adherence to HIPAA guidelines and policies.
  • Monitors, evaluates and maintains knowledge of community-based resources and services necessary to respond to the Member’s individual health care needs, and assists member and family in gaining access to resources.
  • Utilizes proper telephone and personal etiquette and judicious use of other verbal and written communications, following OHSU and OHSU Health Services policies, procedures and guidelines. Follows OHSU and OHSU Health Services telecommuting policies and procedures as a member of the field-based staff.
  • Contributes to the development and evaluation of new programs to meet identified Member needs; provides community education programs to address pertinent social concerns; assumes active role in meetings and care conferences actively contributing to case reviews for the purpose of information exchange, education and enhanced member care planning. Able to service in a supervision and evaluation role of graduate level interns.
  • Ensures day-to-day processes are conducted in accordance with nationally accepted standards (NCQA, URAC and other regulatory standards).
  • Fosters positive and professional relationships and acts as a liaison with internal and external customers to ensure effective working relationships.

Responsibilities
  • Master’s in Social Work required with a Certificate of Clinical Social Work Associate (CSWA) or Licensed Clinical Social Worker (LCSW) required.

  • Expectation to obtain Professional Certification (ACM, CCM, CCP) within 24 months of hire.
  • Clinical assessment skills, consistent with state licensure scope of practice.

  • Ability to assess member psychosocial status, consistent with state licensure scope of practice requirements.
  • Able to prioritize and target interventions, consistent with state licensure scope of practice requirements.

  • Knowledge and experience in working with computer systems and electronic medical records.
  • Able to travel within the tri-county area with an active and unrestricted driver’s license, reliable transportation and active automobile insurance coverage.

Qualifications
  • Minimum two (2) years of clinical experience.

  • Prior experience in case management.

  • Knowledge and experience working within Epic as the electronic medical record.

  • Knowledge of Medicare and Medicaid.

  • Knowledge of adult learning and behavior change principles.

  • Knowledge of community-based resources.

  • Demonstrated skills in motivational and health coaching, patient advocacy, health promotion, disease prevention, patient education and resource management.

  • Demonstrates knowledge of NQCA and Case Management guidelines.

  • Provides care management services appropriate to the age of the member served – neonate, infant, pediatric, adolescent, adult and geriatric patients based on the principles of growth and development and life stages.

  • Professional certification (ACM, CCM, CCP).
