Overview

The RN coordinator provides effective care coordination for pregnant patients with fetal anomalies and genetic conditions. The nurse coordinator is responsible for the active management, including transition management, of patients throughout their care. The nurse coordinator acts as the liaison between referring providers and members of the patient’s care team. The nurse coordinator serves as the identified contact point for patients as they navigate their care within the fetal care program. The nurse coordinator participates in the clinic, department, program, multidisciplinary teams and committees and is expected to initiate and/or actively participate in quality improvement projects as part of a multi-disciplinary team. This position also assists in the implementation of nursing system that supports the mission of the American Academy of Ambulatory Care Nursing (AAACN).

  • Oversees pregnant person referral intake (receipt and review of records, scheduling, and needs assessment)
  • Primary contact point for patients within the fetal care program from referral through delivery.

  • Active participant in maintaining continuity of care across care areas during the perinatal period.
  • Active participant in the multidisciplinary team in the development of individualized, patient centered care.
  • Participates in counseling of pregnant person with fetal team and multidisciplinary specialists
  • Assesses the pregnant person/family for educational and psychosocial needs and refers to social work as necessary.
  • Responsible for handling urgent/emergent calls and participating in provider visits for patients when appropriate.
  • Responsible for supporting prenatal operating room procedures for fetal care patients, from the perspective of care coordination, continuity, and fetal intervention expertise.
  • Supports the pregnant person and family in situations of perinatal loss and/or termination of pregnancy.
  • Participation in research studies and protocols.

Responsibilities
  • Minimum of two years of experience in a perinatal-neonatal department within the last five-years.
  • Current, unencumbered Oregon State Registered Nurse License
  • BLS at time of hire and must be issued by the American Heart Association (AHA)
  • Strong therapeutic communication
  • Ability to prioritize multiple competing tasks in an efficient and complete manner.

Qualifications
  • One year of experience working in a Maternal Fetal Medicine Clinic or other high risk obstetric clinic.
  • National nursing certification in ambulatory nursing or care
  • Experience working with socially and medically complex patients and their
  • Experience in a fetal care center and/or fetal interventions.
