As a Home Health Licensed Practical Nurse (LPN), your responsibilities include:

- Providing proficient nursing services, under the supervision of a Registered Nurse, abiding by the state Nurse Practice Act, any relevant licensure or certification necessities, healthcare standards, and specific rules and regulations alongside adhering to the CenterWell Home Health policies and procedures.
- Applying experienced nursing interventions in the treatment and prevention of the patient/clients illnesses and rehabilitation needs, making use of a holistic approach in the provision of exact therapeutic interventions, teaching, and training activities as per the plan of care.
- Using your knowledge and skills to respond to health issues and achieve personalized patient goals and outcomes.
- Communicating with the RN Supervisor for any necessary changes in the Treatment Plan, receiving verbal orders from physicians, and reporting these orders to the RN Supervisor.
- Ensuring systematic and individualized goal-driven approaches are applied in implementing the nursing plan of care.
- Keeping in touch with patients, physicians, clinical managers, and other members of the healthcare team concerning patient/family needs and status changes, and participating in care coordination activities and discharge planning as needed.
- Upholding the highest professional conduct in relation to confidential information and sharing it only when necessary and in the best interests of the patient.
- Ensuring adherence to professional standards in the provision of and availability of supplies, materials and equipment necessary to safely and efficiently implement the plan of care.
- Preparing, submitting, and maintaining documentation as required by the Company and/or facility with visit/shift notes documented on the day services are rendered.

Key Experience/Skills:

- Completion from an accredited Licensed Practical Nursing Program or an accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current certification in CPR.
- Two years or more of experience working as an LPN/LVN in medical settings, preferably in home health or hospice situations.

The work schedule is 40 hours per week, and the estimated starting base pay for full-time employees (working 40 hours per week) falls between $44,200 and $60,900 per year.
