JOB PURPOSE AND OVERVIEW:

The Home Therapies RN CAP 1 is an initial entry designation into our Clinical Advancement Program (CAP) for registered professional nurses. This role entails being held accountable for providing and overseeing clinically competent care for a selected group of patients. This involves various stages such as assessment, planning, intervention, and evaluation. In adherence with predefined protocols, the role holder will manage and assess patient responses to home dialysis therapy and training, escalating issues to physicians when necessary. As a member of the End Stage Kidney Disease (ESKD) healthcare team, this position encompasses decision-making, teaching, leadership functions, and quality improvement activities aimed at enhancing patient care outcomes and facility operations.

KEY RESPONSIBILITIES:

The individual is expected to act in line with the policies, procedures, nursing standards, federal and state legislation of Fresenius Kidney Care. As part of the role:

- They will perform all critical functions assisted by the Supervisor and guided by the Educator, Preceptor, or another Registered Nurse.
- They will lead the ongoing systematic collection and evaluation of dialysis data for their appointed patients and make necessary treatment modifications.
- The role holder will assess, cooperate, and log patient/family’s fundamental learning needs to provide continuous education.
- In partnership with the patient, home care associate, additional and ancillary patient care staff, they will direct all aspects of safe and effective dialysis therapy provision.
- The role involves administering medications as prescribed and documenting their medical justification and effectiveness.
- The individual will help with emergency response measures when needed.
- They will act as a resource for the healthcare team and take part in staff training and new staff orientation.
- The RN will guarantee the correct completion and shipment of laboratory collections and reschedule any missed or insufficient collections.

The role also involves ensuring patient awareness related to treatment modality options and transplantation, assisting in the identification, evaluation, education, and selection of Home Dialysis candidates and Home Partners. They will conduct assessments of the Home Dialysis candidate’s home environment, partner/family readiness, and evaluate their ability to carry out home dialysis. 

Additional duties will include:

- Training Home Dialysis patients and/or Home Partners to operate Home Dialysis equipment safely and effectively.
- Meeting CAP requirements to progress.
Other duties may also be assigned by the Supervisor.

PHYSICAL DEMANDS OF THE JOB:

This job involves direct patient care that regularly includes heavy lifting and movement of equipment, patients, and helping with ambulation. Assistance may be provided by equipment aids and/or colleagues. The role requires frequent, long periods of standing and bending over. Occasionally, the individual may need to move, with help, machines and equipment of over 200 lbs and lift chemical and water solutions up to 5 feet high, weighing up to 30 lbs. The work environment entails an air temperature-controlled health care facility with moderate noise levels, potential infectious and contagious disease/material exposure. The role will involve participation in an on-call rotation and may require travel.

DAY TO DAY JOB DETAILS:

Daily tasks will involve desk work, computer work, collaboration with patients, and combined facility/hospital staff and physicians. Upon employment, employees are required to take Ishihara's Color Blindness test. However, failing this test does not prevent employment. Reasonable accommodations will be considered.

SUPERVISORY RESPONSIBILITIES:

This role involves oversight of LPNs/LVNs, RNs, Patient Care Technicians, and Home Therapy Care Team Assistants as a Team Leader or designated Nurse in charge. This is after the successful completion of all Fresenius Kidney Care education and training requirements for new employees, a minimum of 12 months of RN experience and 3 months of RN experience in home peritoneal dialysis and/or hemodialysis.

EDUCATIONAL AND LICENSING REQUIREMENTS:

- Accredited School of Nursing graduate
- Current state licensure compliance
- Current or successful completion of CPR BLS Certification
- Must meet state practice requirements

PREFERRED EXPERIENCE AND SKILLS:

- Less than 1 year of Nephrology Nursing experience in the past two years as an RN.
- Minimum of 1 year of experience as a Registered Nurse
- Prior home dialysis therapy experience
