Job Overview
The Case Manager I, HIV position is responsible for a range of tasks that facilitate the client’s ability to engage in and remain in medical care. The Manager engages newly diagnosed patients and linking them with medical care, patient advocacy, assist them in applying for public benefits, and completing applications for the AIDS Drug Assistance Program (ADAP), completing Ryan White eligibilities, providing outreach to clients who are at risk of or who have fallen out of care, scheduling transportation, and completing monthly reports and special projects. The Manager works as an integral part of the Psychosocial Program staff. The specific tasks of the role will vary depending on the program to which the staff member is assigned.
Minimum Requirements:
1. HS Diploma is required.
2. Bachelor’s degree in Human Services or related field is preferred.
3. Minimum of 1 year case management or equivalent work experience within a related field of Health and Human Services strongly preferred.
4. Knowledge of community resources and public assistance benefits.
5. Must be certified through the Los Angeles County HIV Case Management Certification process - to be arranged following employment.
