OBJECTIVE AND SCOPE:

The Outpatient RN CAP 1 is a novice role in the Clinical Advancement Program (CAP) for registered nurses. The holder of this position will be responsible for providing and coordinating proficient care including evaluation, planning, intervention, and assessment for a set group of patients. Tasks related to patient care may be delegated to direct patient care employees such as RNs, LVN/LPNs, and Patient Care Technicians. Being a member of the End Stage Kidney Disease (ESKD) healthcare team, this role involves partaking in decision-making, teaching, leadership functions, and activities for improving patient care results and operational functions of the facility. 

KEY DUTIES AND RESPONSIBILITIES:
All activities and responsibilities ought to conform to Fresenius Kidney Care policy, nursing procedures, state and federal regulations.

- Executes all core duties under the supervision of the Supervisor with guidance from the Clinical Educator, Preceptor and/or cooperation with other Registered Nurses.
- Conducts ongoing data collection and analysis of patient data pre, during, and post hemodialysis sessions for allocated clientele, which is documented in their medical record. Adjustments to the treatment plan are made as deemed necessary.
- Assesses, documents, and provides initial as well as ongoing education to patients and their families.
- In collaboration with direct and ancillary patient care staff, coordinates and directs all aspects of daily delivery of safe and efficient hemodialysis therapy to assigned patients.
- Administers prescribed medications or in accordance with approved approach(s) with justifiable medical reasons and documented effectiveness.
- Initiation or assist with emergency measures.
- Participates in staff training and acts as a resource for the healthcare team.
- Directs correct lab processing and shipping procedures, reschedules missed/sufficient lab procedures.
- Identifies expected results and updates nursing assessment and care plan for assigned patients through cooperation with the Interdisciplinary Team.
- Aware of patients' needs related to treatment modality and transplant options.
- Completion of CAP requirements for progression.
- Performs any other duties assigned by the Supervisor.

PHYSICAL EXPECTATIONS AND WORK SETTINGS:
The physical requirements and workplace atmosphere described here are those that the employee will encounter while performing the fundamental functions of the job. Adaptations may be made to enable individuals with disabilities to perform the necessary duties.

- Requires heavy lifting, moving of patients and helping with walking. Equipment or fellow workers may be available for assistance.
- Frequent, extended standing time is a requirement.
- Occasional machine and equipment moving (up to 200 lbs.) with assistance may be required. It may also involve lifting chemical and water solutions (up to 30 lbs.) up to 5 feet.
- Typical healthcare facility setting with controlled air temperature and moderate noise.
- Exposure to infectious and contagious diseases/material may occur.
- Day-to-day work entails desk work and computer tasks along with engaging with patients, facility/hospital staff and physicians.
- Travel to other facilities or training sites may be needed.
- May have to perform the key duties in different locations under similar working conditions.

SUPERVISION:
The oversight of Patient Care Technicians/LPNs/LVNs/RNs is assigned as a Team Leader or designated nurse after:

- Successfully completing all FKC education and training for new hires.
- A minimum experience of 9 months as a RN.
- A minimum 3-month experience in chronic or acute hemodialysis as a RN.

EDUCATIONAL AND LICENSING REQUIREMENTS:

- Accredited School of Nursing graduate.
- Current state licensure.
- Must finish CPR BLS certification.
- Must fulfill the practice requirements in the state of employment.

NECESSARY EXPERIENCE AND SKILLS:

- Entry-level for RNs with under a year of Nephrology Nursing experience in the previous two years as a RN.
- Minimum 9 months experience as a Registered Nurse (preferred).
- Experience in chronic/acute hemodialysis (preferred).
