ROLE AND RESPONSIBILITIES:

The Outpatient RN CAP 2 role is suited for those who are new to the Clinical Advancement Program (CAP) or those who have advanced from the RN CAP 1 position. This professional registered nurse role is entrusted with the responsibility to ensure and coordinate the provision of clinically proficient care. This includes planning, evaluation, intervention, and assessment for a designated group of patients. Tasks may be delegated to direct patient care staff including RNs, LVN/LPNs, and Patient Care Technicians. As a key player in the End Stage Kidney Disease (ESKD) healthcare team, this professional is involved in decision-making, teaching, and leadership activities, as well as quality improvement tasks that augment patient care outcomes and enhance facility operations.

PRIMARY TASKS AND RESPONSIBILITIES:

All tasks and responsibilities are to be carried out in alignment with policy, procedures, nursing practice standards, and state and federal regulations.

Key responsibilities include:

- Collaboration with other Registered Nurses under the guidance of the Clinical Educator, Preceptor, and Supervisor
- Ongoing data collection and analysis of patient information both pre and post hemodialysis for assigned patients
- Assessment and collaboration with patients/families to provide initial and ongoing education
- Provision and supervision of safe and efficacious delivery of hemodialysis therapy
- Administration and documentation of prescribed medications, providing appropriate medical justification and effectiveness
- Initiation or assistance with emergency response measures
- Role modeling and mentoring for new employees
- Ensuring correct laboratory procedure is followed including collection, processing, and rescheduling of missed or inadequate collections
- Identification of expected outcomes, and updating of nursing assessment and care plan
- Ensuring patient awareness of transplant and treatment modality alternatives.

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

This role necessitates direct patient care which often involves heavy lifting, patient movement, and assistance with ambulation. The employee may occasionally need to move heavy equipment and lift chemical and water solutions of up to 30 lbs. as high as 5 feet. The position requires prolonged standing and bending over. As a healthcare provider, this role may involve exposure to infectious and contagious diseases/materials.

SUPERVISION RESPONSIBILITIES:

The role involves supervising the Patient Care Technicians//LVN/LPNs/RNs as a Team Leader or designated Nurse in Charge, once all FKC training requirements have been competently fulfilled by the new employees.

EDUCATIONAL AND LICENSURE REQUIREMENTS:

Prerequisites include graduation from an accredited School of Nursing and current state licensure. Also, current or successful completion of CPR BLS Certification and the fulfillment of practice requirements in the state of employment.

EXPERIENCE AND REQUIRED SKILLS:
