Position Duties for a Hospice RN/RN Case Manager:
- Reporting directly to the Administrator, Executive Director, or Senior Patient Care Manager.
- Acting as the principal liaison among the physician, patient/family, and hospice team.
- Delivering primary patient care services to hospice patients and support to their families.
- Educating caregivers of patients to empower them.
- Collaborating with an interdisciplinary team of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff.
- Evaluating patient and family needs and assisting in devising and executing the patient care plan.
- Maintaining assigned patient clinical records.
- Participating in company quality assessment and performance improvement programs.
Required Qualifications for the Hospice RN/RN Case Manager:
- Demonstrates compassion and empathy towards patients and their families.
- Ability to provide care in the patient's preferred environment.
- Demonstrates flexibility and composure during crisis situations.
- Possessing an RN license in the state of employment (or eligibility to obtain).
- Minimum 1-year nursing experience in hospice, home health, ICU, geriatric, or oncology.
- Preferred additional experience: Hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry or travel nursing.
- Valid driver’s license and automobile insurance.
- Current CPR certification.
