OVERVIEW:
The Outpatient RN CAP 1 serves as an initiating designation into the Clinical Advancement Program (CAP). This role is entrusted with the task of providing coordinated, clinically effective care including assessment, planning, intervention, and evaluation for a specified patient cluster. This function may involve the assignment of suitable tasks to direct patient care professionals including, but not restricted to RNs, LVN/LPNs, and Patient Care Assistants. As an integral part of the End Stage Kidney Disease (ESKD) medical team, the occupant of this role actively participates in decision-making, teaching, leadership roles, and quality improvement efforts that drive patient care outcomes and the functioning of the infrastructure. 

KEY RESPONSIBILITIES:
The expected responsibilities should be carried out according to the policies, procedures and nursing standards of the organization, and the relevant state and federal legislation.

- Under the oversight of the Supervisor, and with guidance from the Clinical Educator or Preceptor, perform all essential functionalities collaboratively with other Registered Nurses.
- Continuously collect and analyze patient data throughout their hemodialysis treatment process, update these in the patient's medical record, and adjust the treatment plan as required with due notification to the Team Leader, Charge Nurse, Supervisor, or Physician.
- Collaboratively assess, document, and meet the basic learning needs of patients and families through ongoing orientation. 
- In association with direct and ancillary patient care teams, oversee and facilitate the daily safe and effective provision of hemodialysis therapy to assigned patients.
- Prescribed administration of medication in accordance with approved algorithms, documenting medical justification and effectiveness.
- Initiation or assistance with emergency response procedures.
- Serving as a health care team resource, participation in staff training, and orientation of new staff as directed.
- Ensuring correct laboratory collection, processing, and shipping procedures are followed, and rescheduling missed or insufficient laboratory collections.
- Identify and document expected outcomes, collaborate, and update the nursing assessment and plan of care for assigned patients by liaising with the Interdisciplinary Team.
- Raising patient awareness about transplant and treatment modality options.
- Completing CAP requirements for progression.
- All other duties that may be assigned by the Supervisor.

PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT:

The role entails direct patient care involving regular heavy lifting, patient movement, and assistance with ambulation. Intervention by equipment aids or staff may be required. This role necessitates prolonged periods of standing and bending over. The occupant may occasionally need to move, with help, machines and equipment weighing up to 200 lbs, and lift chemical and water solutions weighing up to 30 lbs to a height of 5 feet. The work environment aligns with that of a health care facility with controlled air temperature and moderate noise levels. There may be exposure to infectious and contagious diseases and materials, as well as desk work, computer work, and interaction with patients and medical team members.

SUPERVISION:
Oversight of Patient Care Technicians/LPNs/LVNs/RNs as a Team Leader or designated Nurse in Charge upon fulfilling the below conditions:

- Successful completion of all relevant education and training requirements.
- A minimum of 9 months of RN experience.
- At least 3 months of chronic/acute hemodialysis experience as a RN.

EDUCATION AND LICENSURE REQUIREMENTS:

- Graduation from a recognized School of Nursing.
- Current appropriate state licensure.
- CPR BLS Certification possession or completion.
- Made to meet the practice requirements depending on the state of employment.

EXPERIENCE AND NEEDED YOU SKILLS:

- Less than 1 year of Nephrology Nursing experience as a RN in the last two years are considered entry level.
- A minimum of 9 months of experience as a Registered Nurse (preferred).
- Chronic/acute hemodialysis experience (preferred).
