As a Reputed Nurse in Home Health, your duties will encompass:
- Performing admission procedures, managing patient cases, and conducting subsequent skilled nursing visits for home health patients.
- Implementing seamless care and case management for each patient, providing necessary follow-up as directed by the Clinical Supervisor.
- Collaborating with physicians to formulate an initial care schedule based on their directives and early patient evaluation.
- Delivering practical care, managing, and assessing the patient's care plan and education needs following physician orders, under the supervision of the Clinical Manager.
- Updating the care schedule after discussing with physicians, based on continual assessments and as required by policy and regulations.
- Coordinating all patient care activities, assimilating healthcare professionals such as Physical Therapists, Occupational Therapists, Home Health Aides, and outsourced care providers into the patient's care engagement.
- Regularly reporting patient care/condition/progress to the patient's physician and Clinical Supervisor.
Application of Your Skills for Visible Impact
Mandatory Experience/Skills:
- A Diploma, Associate, or Bachelor’s Degree in Nursing is a must.
- Ideally, a minimum of one year of nursing experience.
- Previous experience in medical-surgical, ICU, ER, or acute care settings is beneficial.
- Familiarity with home health care is a bonus.
- Active and unrestricted Registered Nurse licensure.
- Present CPR certification.
- Exceptional organizational and communication skills.
- Valid driver’s license, automobile insurance, and reliable transportation.

The projected annual salary for full-time positions (40 hours a week) starts from $69,800 to $96,200.
