Youth & Family Services (YFS) help parents and young people who are striving to become healthy and restore their relationships. YFS works in partnership with human services, child welfare systems, juvenile courts, and community-based service providers so that together we can help individuals and families obtain the services and skills they need to live in positive, healthy ways.

We are currently looking for full-time – Intake Coordinator - Bi-lingual preferred

Starting at $43,000 – contingent upon experience, education, etc.


This position will conduct intake and screen clients for appropriate participation in assigned program for referrals to ancillary services and maintain all required documentation. This position will assess individuals referred for mental health and/or substance abuse problems. This includes developing service goals and linkages with schools, and relevant community-based organizations that enhance and ensure clients/ families are in receipt of optimal care and service coordination for addiction intervention, AODA counseling and recovery support services.


Essential Duties and Responsibilities

  • Conduct staffing with referral source and clinical supervisor to address reason for referral prior to acceptance into program.
  • Conduct orientation and intake to coordinate other services, resources and people to assist clients in achieving their established service plan goals.
  • Staff case with clinical supervisor for appropriate case assignment.
  • Complete and maintain all required documentation pertaining to clients (documentation, ECR finding letter and status reports).
  • Facilitate individual orientation to program services or educational sessions (as required)
  • Provide referrals for identified needs of the client and crisis intervention when needed
  • Interview clients, review records, and confer with other professionals to determine their suitability for program participation.
  • Provide specialized care coordination services to clients and their families with behavioral health concerns and other needs which may include addiction, mental health, etc.
  • Maintains a therapeutic alliance with clients through advocacy and resource assistance, including but not limited to medical health needs, housing, vocational services, benefits and entitlements.
  • Intake Coordinators will make follow-up calls to youth and family with weekly outreach which may include texts/emails and home visits;
  • Provide referrals for identified needs of the client and crisis intervention when needed
Requirements:

Our Ideal candidate:

  • Bachelor's degree in human services, social work, psychology or other related fields or two to four years' experience.
  • Knowledge of human behavior for the assessment and signs and symptoms of substance abuse disorders. Specific knowledge necessary for working with special populations.
  • Knowledge of the Code of Ethics for substance abuse professionals
  • Knowledge of the federal drug and alcohol confidentiality law (42 C.F.R. Part 2) and the Health Insurance Portability and Accountability Act
  • Knowledge of the criminal justice system
  • Knowledge of computers, including proficiency in Microsoft Office applications and the Internet
  • Ability to work with a diverse population
  • Ability to work flexible hours and travel as needed (evening/weekend hours)
  • Ability to obtain CADC certification through the State Certification Board (IAODAPCA) within the first two years in the field (if required by program).
  • Must be available to attend meetings and meet the needs of the program and/or clients throughout the city, counties or wherever needed (a valid driver's License, current auto insurance and reliable automotive transportation are required).

The agency currently offers hybrid work schedules that combines in-office and remote work. Employees are required to report to the work site three days per week with the option to work up to two days remotely.
