Job Overview

Key Tasks for the Wellness Nurse:

- Recognize continuous requirements and services of residents to ensure high-quality care. 
- Participate in the evaluation, documentation, and follow-up of residents undergoing changes in their health care services. 
- Collaborate with Resident Care Director (RCD) in resident assessments and gathering vital data. 
- Conduct monthly wellness check-ups for all residents. 
- Monitor and manage documentation for residents who have experienced accidents or alterations in health. 
- Maintain open communication with resident families or responsible parties to coordinate medical care. 
- Contact the resident's doctor when necessary or requested. 
- Perform regular weight and vital sign checks for every resident. 
- Maintain medicinal supplies and emergency kits for the community. 
- Provide clinical support to the community team members as necessary.
- Adhere to and enforce infection control practices.
- Regularly review each resident's medication and treatment profile for accuracy.
- Participate actively in Sunrise Quality Assurance Meetings & Clinical Meetings. 

The ideal candidate will have:

- Graduated from an accredited nursing school. 
- A current state license as an LPN/LVN. 
- At least one year's experience in home health, assisted living, hospital, or long-term care.
- Proven skills in nursing practices applied to health and wellness care. 
- Familiarity with federal, state, local regulations on resident care. 
- Excellent assessment skills. 
- Abilities to handle multiple priorities. 
- Strong written and verbal communication. 
- Good organizational and time management skills. 
- Proficiency in Microsoft Office (Windows, Outlook, Excel) and Sunrise applications.
- Flexibility to work weekends, evenings and unusual hours.
