STAFF NURSE/ 40 HOURS / VARIABLE/ DAY SHIFTS BWH PSYCHIATRY, 221 LONGWOOD AVENUE, BOSTON MA GENERAL SUMMARY/ OVERVIEW STATEMENT - The Registered Nurse (RN) is an integral member of the care delivery team, responsible for supporting the clinical and administrative needs of psychiatry patients, thus ensuring the highest quality of patient care across the care continuum. - Founded on trauma-informed care principles, the model promises to transform practice and the care delivery for psychiatry patients who are undergoing a variety of treatments for the full spectrum of psychological and psychiatric disorders, ranging from major depression and anxiety to chronic psychosis. - In this model, the patient is at the center of the team, and the nurse works collaboratively to manage and coordinate care, triage urgent clinical needs, provide patient education, administer medications, offer telephonic support, participate in prescription management and other administrative duties. ADDITIONAL DUTIES AND RESPONSIBILITIES: 1. Creates an authentic healing environment that supports the well-being of patients and their families. Supports the Department’s Call Center and Clinical Triage Team to support triage efforts of urgent patient needs. 2. Ensures a seamless patient flow and positive patient experience across the care continuum in coordination of the individual practices throughout the d epartment and its satellite locations. Works directly with the prescribers, clinical, and administrative teams as well as external agencies such as pharmacies, insurances and v isiting nurse agencies to resolve patient issues and concerns. 3. Prepares, performs, and documents the coordination of clinical care, including assisting the patient with insurance and clinical components of paperwork, assisting with prescription refills, and other services to support the prescribers and other team members. 4. C ollaborat es with other team members on prior authorizations for prescriptions. Participates in the medication reconciliation process including medication review. 5. Provides p atient education as needed and requested. Develop s and maintain s patient education materials in coordination with the department's clinical team. 6. Follow up telephone calls and other electronic communication regarding clinical questions as requested by provider s 7. Participates in patient-centered health-maintenance efforts, i ncluding coaching patients towards health/wellness, disseminating information on condition, discharge medications, and follow-up care, which may include leading or participating in educational groups and hosting health-based educational series. 8. Co llaborates with attending physicians, nurse practitioners, social workers, psychologists, community health workers, practice assistants and other team members as appropriate. 9. Assists the attending physicians and other prescribers in any potential collection and analysis of validated quality and outcome measure data. Collaborates on projects that impact overall departmental goals and initiatives. 10. May also participate in related research projects after completion of the required training. Operations Support/Administrative Responsibilities: 0. Remains proficient with BWH computer systems as needed to independently perform daily tasks such as order entry, EMAR, lab tests and results, incident reporting, etc. 1. Consistently provides high quality and timely documentation, including admission notes, progress notes, telephone encounters, discharge summaries and dictations. 2. Participates in departmental education and training to medical assistants, other staff members and students regarding th e needs of the d epartment. Other duties as assigned by the supervis or/manager or leadership team. QUALIFICATIONS 1. Graduate of an approved school of nursing with current registration in nursing in Massachusetts. BSN is required for newly licensed nurses. 2. Must have 2 years’ recent psychiatric experience with a working knowledge of both ambulatory care needs as well as the inpatient discharge process preferred. 3. Previous Psychiatric ambulatory experience with a demonstrated ability to educate patients and families around current treatments and medications preferred. 4. Demonstrates true enthusiasm to provide nursing care for patients and families. 5. Experience working in a large complex health care organization. 6. Proven ability to achieve programmatic goals and outcomes demonstrating impeccable organizational skills. Ability to work with various information systems and databases: Teams, MS Word, PowerPoint, Excel, Access and others. 7. Demonstrate s ability to work collaboratively with clinical and administrative staff Given the importance of this role to the overall success of Psychiatric programs and patient care initiatives, candidate must have the ability to function at the highest level professionally and clinically and further possess excellent interpersonal skills. WORKING CONDITIONS - The work may require lifting heavy objects and patients and exposure to potentially infective material. Work requires contact with needles and other sharp instruments, monitoring devices, and other state-of-the-art equipment. Prolonged standing may be required. The hospital is approximately ¼ mile long and excessive walking to and from the patient care units can be expected.
