Care Management Social Work provides psychosocial/emotional support to patients and families.
Clinicians complete psychosocial assessments and identify appropriate discharge plans from a person centered approach and in alignment with policy and regulations.
Social Work Clinicians provide brief clinical interventions as appropriate, assisting with resource navigation and coordination of care.
SW is split into three areas: Emergency Department, Children's, and Adult teams.

Alaska Social Work license NOT required

Shift: 5x8 Days (40Hrs)
Guaranteed Hours: 72 Hours per 2 weeks
Traveler can be called off for 1, 8-hour shift per 2 weeks
